What is the Comprehensive Community Services Program?

The Comprehensive Community Services Program (CCS) is a voluntary program available to all residents of Forest, Oneida and Vilas counties. CCS is a psychosocial rehabilitation program for all ages, serving individuals with substance abuse, mental health, or co-occurring diagnoses.

Some of the more common mental health diagnoses treated include, but are not limited to, Anxiety, Depression, Post Traumatic Stress Disorder (PTSD), bipolar disorder and Attention-deficit/hyperactivity disorder (ADHD).

CCS works with individuals and their team to decide which programs and support will help the individual reach their recovery goals and their highest potential while living in their community. No two treatment plans look the same because they are tailored to each client, to address every aspect of the individual’s life touched or affected by their mental health or substance use diagnosis

Who is eligible for the Comprehensive Community Services program?

  • People of all ages who life with a mental illness, a substance use disorder or both.
  • Must be a resident of Oneida, Forest, or Vilas County.
  • Qualify for CCS services as determined by a functional screen.

Who is in the Comprehensive Community Services Team?

  • Client

    The individual who has completed the admissions process and is receiving treatment and services.

  • Natural Supports

    Anyone that the client feels would be beneficial in their recovery, often family, friends, teachers, mentors, etc.

  • Service Facilitator

    HSC employee who coordinates and leads the client's recovery team. Performs outreach to members of the team and maintains regular contact with client to monitor their progress.

  • Clinical Coordinator

    HSC employee who reviews, provides input to and approves a clients treatment plan.

  • Other professionals are selected based on the client's unique needs.

    Some examples of other workers include psychotherapists, direct care workers, dieticians, or any other professionals appropriate for the client’s recovery.

What services can the Comprehensive Community Services program provide?

CCS Service Arrays include the following services:

  • Service Planning

    Person centered approach to develop goals in collaboration with the client and their service team based on what the client wants to achieve. Written plan of services that will be provided to the client based on their needs and reviewed on an ongoing basis.

  • Service Facilitation

    Ensures coordination for the services needed. Includes advocacy and delivery of services in a timely manner.

  • Medication Management

    Diagnosing, prescribing medications, monitoring symptoms and side effects.

  • Physical Health Monitoring

    Monitoring of the client’s activities and management of their physical health and wellness.

  • Peer Support

    Certified Peer Support Specialist is an individual with lived experience and has been trained to provide guidance who will work as an equal with the client toward living in recovery.

  • Individual Skill Development and Enhancement

    Through identification of specific needs, skill building can occur but not limited through modeling, monitoring, cuing, and assisting.

  • Individual and/or Family Psychoeducation

    Education about the client’s diagnosis, including identifying resources, building skills, providing ongoing guidance and support. May be held individually or in a group setting.

  • Wellness Management and Recovery/Recovery Support Services

    Helping the client take a broader look at their life and provide support in the areas that may be affected by their diagnosis.

  • Psychotherapy

    CCS contracted therapists meet with clients to help the client modify their behaviors, cognitions and emotions. This can be held individually or through a therapy group.

  • Substance Abuse Treatment

    Substance abuse treatment may be held individually or if offered in a group setting.

How Can I Get More Information?

An adult or child can be referred to CCS by completing the referral form. Parents, schools or providers may refer someone to CCS, or an individual can self-refer.
