Our individualized services help adults define and achieve their recovery goals. Our team meets you where you are, listens to your needs and preferences, and helps you identify the steps to take toward recovery. We could use a team-based approach to collaborate with you, your providers, and your supports. Or, if that approach is not a good fit, we can work with you individually on your recovery goals. We offer a range of counseling, psychiatry, and case management services and work to protect your rights and advocate for you.
The Adult Community Service Programs offered at The Human Service Center vary depending on intensity of need, diagnosis, and funding source. The different case management programs offered pending medical need are Targeted Case Management (TCM), Comprehensive Community Services (CCS), and the Community Support Program (CSP) benefit.
Targeted Case Management tends to be a short-term program (6 months- 1 year) to assist an individual with major mental illness for reasons that pertain to their mental illness that has interfered in different scopes of life by getting established in mental health services and identifying resources to meet their needs. An individual would receive a service facilitator that they would generally meet with monthly by phone and every 3 months face to face to go over needs and evaluate where the individual is at in meeting their goals.
Comprehensive Community Services is a Medicaid funded program that services individuals with major mental illness. This is a voluntary program that provides mental health and substance use services to individuals of all ages. This is a team based, consumer driven service. This service is recovery based and the individual needs to be able to identify the goals that they would like to work on; in order to reach their highest level of independence per their perspective.

Community Support Program is also a Medicaid funded program that services individuals with chronic and persistent mental illness. This is a program that services individuals 18 years and older. This program can be utilized voluntarily and on an involuntary basis, per court order. This program provides a range of treatment and rehabilitation support services. This program has been identified as a “hospital without walls” as the services in the program are required to be provided by the internal providers of The Human Service center. This program serves individuals that have reduced functioning of daily living ability, difficulty coping with the demands of life, and may lead to an inability to maintain mental health stability and living independently without the long-term treatment and support.

Anyone can be referred to The Human Service Center by completing the attached referral form. An individual can be referred by a Provider, family member, or an individual can self-refer.
